Social Service Assistant – Bi-lingual – (Spanish & English) – Department of Human Services – Division of Senior Services
Under direction, performs paraprofessional work involved in the operations of a social service program; gathers and records significant social, financial, and pertinent information about their clients and/or implements selected aspects of the program; does other related duties as required.
Knowledge and Abilities:
- Obtains and processes new client applications for home delivered meal service including nutrition screening; enters data in State mandated SAMS computer program
- Responds to telephone inquiries regarding Meals on Wheels program in a timely manner and ensures information received is accurate
- Fields incoming telephone calls from clients to suspend/resume service
- Handles client comments and complaints calmly and politely; ensures that client issues are sufficiently resolved
- Replies immediately to driver questions, follows up and assures well-being of clients who did not answer the door upon attempted delivery by MOW driver, contacts local police for wellbeing check when necessary
- Updates driver route listings daily to ensure accurate delivery information and client satisfaction
- Conducts semi-annual reassessments including nutrition screening; enters data, assess results, makes appropriate referral based on assessment
- Supports coworkers with SFMNP (Farmer’s Market vouchers), congregate nutrition and HealthEASE wellness programs
- Gives basic instruction to clients in such matters as menu planning, meal preparation, determining caloric content of various foods, and food preservation
- Participates in community health fairs and elicits prospective client’s enrollment in the nutritional program
- Assists dietitians in counseling clients regarding food safety to improve their nutritional status
- Performs other tasks and duties as assigned by supervisor
Schedule:
- 40 hours per week, Monday – Friday
Education Requirements:
- Associate degree from an accredited college with a major course of study in nutrition care or dietetics
Other Requirements:
- Bilingual Spanish/English – must be able to read, write, speak, understand, and communicate in Spanish and English.
